  • Provides alternatives to hospitalization by providing community treatment orders 
  • Provides community based treatment, care and supervision  
  • Offers individualized support program to manage psychiatric illness
Services include: 
  • Mental health follow-up 
  • Symptom monitoring and support 
  • Assistance to obtain affordable housing 
  • Crisis support 
  • Financial assistance and budgeting support 
  • Interpersonal support 
  • Medication monitoring 
  • Rehabilitation 
  • Relapse prevention
Hours CTO Coordinator Mon-Fri 9 am-5 pm 
CTO Case Manager Mon-Fri 8:30 am-4:30 pm

Eligibility Ages: 18 year(s) and up 
Individuals with serious mental illness who stabilize with treatment in hospital, and after discharge stop treatment, care and supervision resulting in relapse of illness and readmission to a psychiatric facility
  • Referral from psychiatrist preferred 
  • Referral from health care providers, clients, family and friends also accepted 
  • Assessment process required
