Description (Service) Provides a multi-disciplinary team including representatives from social work, pharmacy, nursing, dietitian, family medicine, and the Alzheimer’s Society to address memory issues
 
Services provided include: 
  • Assessment of issues such as delirium and depression based on a clinical reasoning model 
  • Assessment of functional and cognitive impairment 
  • Considerations in individual's fitness to drive
 
Interventions may include but are not limited to: 
  • Medication changes 
  • Referrals to programs and support 
  • Driving status considerations
Hours Office: Mon-Fri 8 am-4:30 pm
Areas Served Kenora ; Lake of the Woods ; Minaki ; Nestor Falls ; Redditt Township ; Sioux Narrows
Boundaries Also includes neighbouring First Nations communities
Eligibility Individuals having memory issues
